This is the form to be used when a student goes for a physical.  Must be completed by a health care provider.

This form is to be completed by the student’s parent or guardian.

This form is required if a student needs to be medicated while at school. Permission must be granted for all medications, including over-the-counter remedies and cough drops.  The form must be signed by a health care provider.

Use this form if your student is authorized to take medication themselves.  This includes epipens and inhalers.  It must be signed by a health care provider.

Have your dentist fill out this form (and return it to school) to certify that your student is in good dental health.
